Targeted Case Management for Adults

This Adult Services program provides mental health case management to individuals with mental health needs. This program aims to assist through advocacy, education, connection to community resources, and support maintaining health care treatment to make sure you experience a better quality of life.

 

Case Managers work with you to determine your mental health, economic, health care, and engagement goals. Once you know what you want to accomplish, your case manager will connect you to helpful community resources, attend medical appointments with you to help you understand and do what the doctor says, connect you to other services to assist with your basic needs, secure housing, and apply for benefits.
